- You will then be e-mailed an intake packet with a comprehensive questionaire to be filled out and returned. Or, you may download and print the Intake Packet from the link above (PDF file).
- Once all forms are received, you will be contacted via phone and/or email to address questions and concerns, to identify whether further assessment is needed, and to schedule an appointment.
- Next, a comprehensive evaluation is completed at our clinic.
- And last, a treatment program is developed to meet your child's individual needs.
